Nourish - Nutrtional Therapy Process

Step 1 – Health Questionnaire

Before booking an appointment, I will ask you to complete a Nourish Health Questionnaire. See the 'forms' page to download, or it can be emailed or posted to you. You will need to return this to me via email or by post to Nourish, Unit 3, 20 Caskgate Street, Gainsborough, DN21 2DL.

Once I am in receipt of your questionnaire, I will contact you to make an appointment for your Initial Consultation. As the information on this questionnaire is fundamental to the consultation, it is not possible for an appointment to be made without it.

Filling out the question should be seen as part of the therapy process. It is a little involved, so make yourself a cuppa, and give yourself the time you need to complete it (about 30 minutes). The form will prompt you to think about all of your symptoms, not just the main ones. Everything is connected, so the more detail you give here, the more I will be able to piece together what is happening with you.

Step 2 – Food diary

Understanding what you eat and drink and when you do so, tells me a lot about your health. Your cravings help me recognise your deficiencies, the times of day you refuel help me understand your metabolic stressors and blood sugar dips. The foods that you avoid help me link symptoms to nutritional imbalances.

The more information you provide here, the more I can help you make changes that won’t be too difficult to follow. Guidance on how to complete a food diary is available here. Ideally, I would like to see two weeks of a completed diary, but one fully complete week is better than two partially completed weeks.

Step 3 – Initial Consultation 1 and 1/2 hours

During this in-person clinic session we will discuss your symptoms, health history, diet, lifestyle, sleep quality, general habits and behaviours, the illnesses experienced by your immediate family, and even the health of your mother whilst she was pregnant with you. A lot of information will pass between the two of us, this can feel a little overwhelming; but you will hopefully leave the session feeling a little lighter.

A personalised nutritional therapy plan will then be emailed to you including suggested dietary and lifestyle changes, supplements, and functional testing recommendations (if appropriate). I will arrange a 10 minute follow up phone call with you after a week or two, to discuss any questions that you may have with your plan.
