Health Questionnaire

Before your consulation appointment can be booked, I will need to receive your completed Health Questionnaire. As the information on this questionnaire is fundamental to the consultation, it is not possible for an appointment to be made without it.

The form is available as a word document for you to download, fill out and then return to me by either email or post. If you are having any trouble accessing the form please request a copy to be emailed or posted to you.

Once I am in receipt of your questionnaire, I will contact you to make an appointment for your Initial Consultation.

Completing a Food Diary Guidance

Please see the attached notes for completing a food diary, and suggested template. If you want to use your own template that is perfectly fine, I just need to see what you eat and when you eat it.

Payments

When an appointment has been made for you, you will be given 48 hours to make your payment. A confirmation text or email will be sent to you to confirm payment has been received. If you do not make your payment within 48 hours, it will be assumed that you have changed your mind, the appointment will be cancelled, and the time will be offered to another person.

Refunds

A full refund can be requested up to 2 weeks before your appointment time.

Within 2 weeks but more than 48 hours before the appointment, a 50% refund can be requested.

Within 48 hours of your appointment, no refund will be given.

Any payment made for lab testing, can not be refunded.

Other

During your consulation you will be asked to sign the following forms:

Consents form
Privacy notice (short form)
Terms of engagement
